What is an AMOSUP Identification Card
AMOSUP identification card (ID) is used in any of AMOSUP establishment such as hospitals, Sailors’ Home, maritime library, Slop Chest, recreation hall, Seamen’s Center, Seamen’s Village, training center and Provident Fund
The Records Department is in charge of the safekeeping of AMOSUP members’ 201 Files. Below are the functions of the Records Department:
- Acquisition and processing of records of new members
- Issuance of AMOSUP ID
- Verifying member’s information
- Keeping the records up to date

Who are qualified AMOSUP members?
All Filipino seafarer onboard a vessel enrolled under AMOSUP are bonafide member and must acquire an AMOSUP identification card (ID).Where can I get an AMOSUP identification card?
Active member can apply for AMOSUP identification card at Records Dept., Seamen’s Center Bldg., Intramuros, Manila or at AMOSUP Seamen’s Hospital Davao, AMOSUP Seamen’s Hospital Cebu and AMOSUP Seaman’s Hospital- Iloilo.
What is Verification?
Verification is an approved clearance from AMOSUP Records Department to ensure the dependent or the member is a bona fide member of the Union and to inform hospitals medical records department of the total registered vessel of the individual member covered by the Union. Members and dependent are always reminded that the verification must be secured first with AMOSUP Records Department.
Who Can Secure Verification?
- Dependents who can not present their Allotment pay slip and Dependents ID to the hospital.
- Members who can not present their Seamen’s registration book (Seaman’s Book) and membership ID to the hospital
How Often Should I Update My Records?
Members who board vessels with an active AMOSUP CBA are reminded to report to AMOSUP Records Department to update their records upon disembarkation.
What are the Requirements?
For members who downloaded the AMOSUP membership forms via website. Please submit your completely filled out forms with attached photocopy of any of the following:
- Seamen’s Registration Book first page with your name and the last page with vessels name you boarded and date of disembarkation
- Certificate of employment from the manning agent
To Records Department AMOSUP Seamen’s Center Building Cabildo Corner Sta. Potenciana Sts. Intramuros Manila for encoding. Submission of form is from Monday to Friday 8:00 am to 5:00 pm and on Saturdays from 8:00 am to 12:00 pm.

Can I still get a medical benefit to Amosup? Even though I don't have yet an assignment letter and I haven't joined any vessel for almost 2 years. However I am already a bonafide member of Amosup for 10 consecutive years. Because I have an medical condition which I needed the assistance of Amosup.
